Here's the Thailand Visa application form I had to fill out while at the Thai Consulate in Vientiane, Laos. The form at any consulate or embassy will be similar but perhaps not identical. Someone who works at the consulate has demonstrated in pink pen how to complete it correctly. Notice at the bottom of the part 4 section where the form asks "Place of Stay in Thailand'. Below this form is another visa application form, this one given out at the Thai Consulate in Saigon (Ho Chi Minh City), Vietnam.
The form below was given to me to fill out at the Thai Consulate in Saigon, Ho Chi Minh City. If you're just applying for a tourist visa, you won't need to fill out the 'Name and Address of Local Guarantor' sections. The address of the Thai Consulate in Saigon is: 77 Tran Quoc Thao Street, Open: From 8.30am to 11.30am in the morning. From 1:30pm to 4:30pm in the afternoon (Monday to Friday). You can apply any time during these hours. I was told I could pick my passport up the following day, after 3pm. Tel: +84 (0)8 3932-7637 / 3932 7638 It's about 1 and a half to 2 kilometers from the backpacker, cheap hotel area of Pham Ngu Lao and the Dong Khoi area, where you'll find more luxurious hotels. Motorbike taxis should cost about 15,000 to 20,000 Vietnamese Dong (VND) (about $1 US), one-way. You cannot get double entry visas from Saigon.
